The request for an inspection must be made by completing this
form and submitting it to the department, a registered accessibility
specialist, or contract provider not later than 30 calendar days
after the completion of construction. The completed Request for
Inspection form must be received prior to proceeding with the
inspection.

Building or facility owners or the owners’ designated agent may
use this form to indicate the status of outstanding violations
associated with the referenced construction project that were
identified during the inspection performed by a Registered
Accessibility Specialist (RAS) or TDLR Investigator to verify
compliance with the Texas Accessibility Standards (TAS).

In accordance with the Texas Department of Licensing and
Regulation’s (TDLR) Administrative Rule 68.10(10), individuals
designated to act on the owner’s behalf must be designated in
writing.

An Elimination of Architectural Barriers Project Registration
form or Architectural Barriers Project Registration Confirmation
Page must be completed for each subject building or facility and
submitted along with the applicable fees when the design
professional or owner submits the construction documents.

In accordance with 16 Texas Administrative Code Texas
Administrative Rules Chapter 68.50(a), “an architect, interior
designer, landscape architect, or engineer with overall
responsibility for the design of a building or facility subject to
§469.101 of the Act, shall mail, ship, or hand-deliver the
construction documents along with a Proof of Submission form not
later than the fifth day after the plans and specifications are
issued.

A separate variance application for each (non-compliant)
condition within a single building or facility must be submitted by
the owner (or owner’s designated agent) and must include a $175.00
non-refundable fee and a Project Registration Form if the project
has not been registered.
